- Submit a Written Request: Include sufficient details to help locate your records.
- Send to the Appropriate Department: Direct your request to the relevant service or department within Mens Mental Health Counselling Therapy.
- Response Time: We will respond to your request within 30 days, with possible extensions as required under PHIPA.
Fees for Access Requests
- Public Health, Long-Term Care, Adult Day Services: $30 for administrative fees, plus $0.25 per page after the first 20 pages.
- Paramedic Services: $75 administrative fee.
In cases of financial hardship, fees may be waived.
